29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about March 15, 2017, at the above located workplace, a machine operator was required to work with a metal cutting lathe. The lathe did not have a rotating chuck guard. The unguarded rotating chuck exposed the employee to a struck-by, caught-in and a struck against hazard.

29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycle: a) On or about March 15, 2017, at the above located workplace, employees were required to work with a Allsteel, Verson press brake. The press brake did not have a point of operation guard. The employees were thereby exposed to an amputation and crushing hazard.
