29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  a. At the worksite where employees worked below employees on the roof who were installing roof sheeting, hard hats were not worn or required to be worn, thereby exposing employees to struck-by hazards.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.  a. In the north stairwell entrance on the first floor where employees accessed the building, holes in the floor around the steel beams were not covered, thereby exposing employees to fall hazards.  b. In the first floor laundry room where employees walked through the building, a floor hole was not covered, thereby exposing employees to fall hazards.  c. In the South stairwell entrance on the first floor where employees accessed the building, holes in the floor around the steel beams were not covered, thereby exposing employees to fall hazards.

29 CFR 1926.1052(b)(1): Except during stairway construction, foot traffic is prohibited on stairways with pan stairs where the treads and/or landings are to be filled in with concrete or other material at a later date, unless the stairs are temporarily fitted with wood or other solid material at least to the top edge of each pan. Such temporary treads and landings shall be replaced when worn below the level of the top edge of the pan.  a. In the north stairway where employees accessed the upper floors, the metal pan stairway treads were not effectively filled in, thereby exposing employees to fall hazards.  b. In the south stairway where employees accessed the upper floors, the metal pan stairway treads were not effectively filled in, thereby exposing employees to fall hazards.
