29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: Note: If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals should have been changed accordingly. On or about April 28, 2016 the employer was using an XTREME XRM 1045 forklift to lift a wood trash bucket as an addition to the lift that created an unstable load that was not specifically approved by the manufacturer of the lift.
