29 CFR 1926.453(b)(2)(iv): Employees did not stand firmly on the floor of the basket, and shall not climb on the edge of the basket or use planks, ladders, or other devices for a work position. West side of the building structure; an employee stood on the JLG Lift, model 600AJ, bumper #703791RA basket rail while working from inside of the basket, exposing employees to fall hazards at approximately 18 feet.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift. West side of the building structure; an employee installed 4 glass windows while working from a JLG Lift, model 600AJ, bumper #703791RA, and was not tied off while inside of the basket, exposing employees to fall hazards at approximately 18 feet. BINSWANGER GLASS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.453(b)(2)(V) CONTAINED IN OSHA INSPECTION NUMBER 314280512, CITATION NUMBER 1, ITEM NUMBER 1, WITH RESPECT TO A WORKPLACE LOCATED AT 6805 NE Loop 820, North Richland Hills, TX 76180. THIS FILE BECAME A FINAL ORDER ON SEPTEMBER 09, 2011.
