29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      a) Facility Wide -  On or about August 14, 2014, the employer failed to establish a program that included energy control procedures for employees to follow, employee training, and periodic inspections to protect employees performing set up and maintenance/servicing on equipment such as band saws (wood pallet dismantlers), trim saws, and chop saws from injury caused by unexpected start up and energy releases.     In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.213(c)(1): Circular hand-fed ripsaw(s) were not guarded by an automatically adjusting hood, which completely enclosed that portion of the saw above the table and above the material being cut:     a) Shop Area -  On or about August 14, 2014, the Dewalt table ripsaw (s/n 2013 49-CT 026734) was used without an automatically adjusting hood guard, which completely enclosed that portion of the saw above the table and above the material being cut.     In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.213(g)(1): Swing cutoff saw(s), including sliding cutoff saw(s), were not provided with an automatically adjusting hood that completely enclosed the upper half of the saw, the arbor end, and the point of operation at all positions of the saw:     a) Shop Area (Chop Saw Area) - On or about August 14, 2014, sliding cutoff saws, including chop saw #02 (Morgan Chop Saw) and the unmarked red chop saw #03, were not provided with automatically adjusting hoods that completely enclosed the point of operation at all positions which potentially exposed employees hands to lacerations and/or amputations.    In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification.
