29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: On or about August 5, 2014, Contractors Access Equipment, Inc. did not ensure that erectors and inspectors of the Sky Climber Rolling Roof Rig were provided with proper instructions for the proper assembly of the supplied rolling roof rigging equipment, including but not limited to the correct placement of pins in the outrigger beams, exposing employees to the hazards of falls.
