Indiana Avenue, Chicago, IL - an employee required to perform scaffolding/ canopy dismantling was not provided adequate training to recognize unsafe working conditions in the work environment, thereby exposing the employee to fall and struck by hazards.

Indiana Avenue, Chicago, IL - an employee performing scaffolding/canopy dismantling was not provided a safe means of access to the work elevation, thereby exposing the employee to fall hazards.
