29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  (a) West side of building at 14049 Scenic Hwy, Lookout Mountain, GA, Covenant College at Carter Hall: Employees working on a system scaffold were not trained on the hazards associated with the scaffold such as, but not limited to; properly installing and working from stair case treads used for access and egress. An employee was assembling the steps on a scaffold approximately 16 feet above ground when a tread became dislodged and the employee fell sustaining injuries to his right arm.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.  (a) The employer did not report a work-related in-patient hospitalization of an employee to OSHA within twenty-four (24) hours of occurrence. An employee injured his upper right elbow when he fell while installing the treads on a system scaffold. The injury occurred on January 12, 2016 and was reported on January 25, 2016.
