Employees were exposed to crushing and amputation hazards from ingoing nip points while engaged in wood cutting activities.

Employees operating the swing saw, the resaw, and others working in the area were exposed to crushing and amputation hazards from the ingoing nip point created by the belt and pulley system.      b)  A & D Wood Products, Inc., Elida, Ohio:  On or about September 5, 2014, the employer did not ensure that three drive pulleys equipped on the MMT manufactured notching saw were guarded.  Employees engaged in saw dust cleanup activities were exposed to crushing and amputation hazards created by ingoing nip points from the belt and pulley system.
