29 CFR 1910.213(g)(1): Swing cutoff saw(s) are not provided with an automatically adjusting hood that completely encloses the upper half of the saw, the arbor end, and the point of operation at all positions of the saw:    The employer does not ensure that employees are protected from amputation injury when operating the H&A Saws, Inc. manufactured swing cutoff saw (model 55; s/n 8519).  This violation reportedly occurred on or about October 4, 2016, when employees operated the saw without an automatically adjusting hood.      To abate this violation, the employer must ensure the saw is guarded to protect employees.

29 CFR 1910.213(i)(1): Non-working portion(s) of the blade of bandsaw(s) are not enclosed or guarded:    The employer does not ensure that employees are protected from laceration and amputation hazards created by the unused portion of the blade on the Baker Products Band Resaw.  This violation reportedly occurred on or about October 5, 2016, when employees operated and cleaned around the saw during a time the discharge chute was removed, exposing them to the unused portion of the blade.    To abate this violation, the employer must ensure the unused portion of the blade is guarded to protect employees.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform are not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    a. The employer does not ensure employees are protected from hazards created by rotating parts in that a pulley, used to drive the resaw feed conveyor, is not guarded.  This violation reportedly occurred on or about September 28, 2016, when employees worked in the area to operate the swing saw and resaw.      b. The employer does not ensure that employees are protected from crushing and amputation injury in that three drive pulleys equipped on the MMT manufactured notching saw are not guarded.  This violation reportedly occurred on or about October 5, 2016, when employees were exposed to the moving parts while cleaning dust from around the machine.    To abate this violation, the employer must ensure belts and pulleys are guarded to protect employees.

This violation reportedly occurred on or about October 5, 2016, when employees working in the area and those operating the swing saw, were exposed to hazards created by the rotating parts. A&D Wood Products, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.147(c)(1), which was contained in OSHA inspection number 108854, citation number 1, item number 3 and was affirmed as a final order on May 3, 2012, with respect to a workplace located at 112 Ward St., Elida, Ohio 45807.    A&D Wood Products, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 991395, citation number 3, item number 1a and was affirmed as a final order on May 9, 2016, with respect to a workplace located at 112 Ward St., Elida, Ohio 45807.

This violation reportedly occurred on or about October 4, 2016, when an employee was exposed to amputation and laceration hazards while changing the machine's blade.    To abate this violation, the employer must ensure that hazardous energy sources are isolated by all employees performing maintenance activities.
