The employer had a main circuit breaker panel located inside of a metal corrugated building.  The circuit breakers inside of the panel were not labeled or marked to identify its purpose, nor was it arranged so the purpose was evident, exposing employees to electrical and fire hazards.

A main breaker panel in the metal corrugated building did not have a clear working space in front, with items being stored directly in front of the panel.  This condition exposed employees to electrical and fire hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  On or about August 26, 2014 and times prior thereto, employees of Scott Stewart DBA Slabtown Customs assembled small homes at or near 602 East Webb Street in Mountain View, Arkansas.  The employer had flexible cords and cables not visually inspected before use for external defects and for evidence of possible internal damage in the following instances:  a. A flexible extension cord was used inside a home being assembled outdoors and was plugged into a radio.  The flexible cord had a tear in the sheathing with visible insulated conductors observed, exposing employees to electrical shock.  b. A flexible extension cord was in use, plugged into other extension cords for a table saw.  The flexible cord had torn sheathing, exposing employees to electrical shock.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  On or about August 26, 2014 and times prior thereto, employees of Scott Stewart DBA Slabtown Customs assembled small homes at or near 602 East Webb Street in Mountain View, Arkansas.  Two outlet boxes that had flexible cords plugged into them were located below a wood working table inside the metal corrugated building and did not have a cover, exposing employees to electrical hazards.  Scott Stewart DBA Slabtown Customs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(2)(i), which was contained in OSHA inspection number 917490, citation number 1, item number 1b and was affirmed as a final order on August 22, 2013 with a final abatement date of August 8, 2014, with respect to a workplace located at 602 East Webb Street in Mountain View, Arkansas.

The employer had installed and used flexible cables manufactured by Romex as a substitute for fixed wiring.  The flexible cords had been found installed at the main breaker panel and fed to outlet boxes and light switches.  This condition exposed employees to fire hazards.  Scott Stewart DBA Slabtown Customs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 917490, citation number 1, item number 2 and was affirmed as a final order on August 22, 2014 with a final abatement date of September 13, 2013, with respect to a workplace located at 602 East Webb Street in Mountain View, Arkansas.
