29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) On or about 8/04/14, on a roof tear-off construction site at 170 Prospect Ave.,  Buffalo, NY:  An employee was exposed to a potential fall of approx. 20 ft.  from a platform at the hoist area of a tubular welded frame scaffold erected on the front side of the building.  There was no midrail on the east half of the north endframe of the scaffold.   b) On or about 8/04/14, on a roof tear-off construction site at 170 Prospect Ave.,  Buffalo, NY:  An employee was exposed to a potential fall of approx. 20 ft.  from a single scaffold pik  running parallel to the south side of the house and spanning  the gap between scaffolds erected on the front and back sides of the house.  There was no midrail on the south side of the scaffold pik. Additionally , there was an approx. 18 in. wide opening  created  at the southeast  corner of the back scaffold between  the scaffold pik and the upright on the endframe where was no guardrail at all.  c) On or about 8/04/14, on a roof tear-off construction site at 170 Prospect Ave.,  Buffalo, NY:  An employee was exposed to a potential fall of approx. 20 ft.  from the west side of the scaffold  platform of a tubular welded frame scaffold erected on the back side of the building.  There was no midrail on the west side of the scaffold platform.  d) On or about 8/04/14, on a roof tear-off construction site at 170 Prospect Ave.,  Buffalo, NY:  An employee was exposed to a potential fall of approx. 20 ft.  from a platform of a tubular welded frame scaffold erected on the back side of the building.  There was no toprail on the west half of the south endframe of the scaffold.   NO ABATEMENT CERTIFICATION REQUIRED
