29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   Worksite located at 2337 N Main St. New Castle OK, 73065. On or about August 25, 2014 and at times prior there to, employees were observed working from scaffolding at a height of more than 10 feet without the use of fall protection.  The employees were exposed to a fall hazard of approximately 42 feet.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph:   Worksite located at 2337 N Main St. New Castle OK, 73065. On or about August 25, 2014 and at times prior there to, employees were observed working from scaffolding that was six sections tall, exceeding the height to base width ratio of four to one (4:1) and was not protected from tipping by the use of guys, ties, or bracing. The employees were exposed to a fall hazard of approximately 42 feet.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   Worksite located at 2337 N Main St. New Castle OK, 73065. On or about August 25, 2014 and at times prior there to, platforms on the working levels of the fabricated frame scaffolding were not fully planked between the front uprights and the guardrail supports.  The employees were exposed to a fall hazard greater than 10 feet.

On or about August 25, 2014 and at times prior there to,  employees were working from scaffolding platforms that ends extended over its support more than 18 inches.  The employees were exposed to a fall hazard of greater than 10 feet.
