The scaffold height to base width ratio was more than 4:1 and employees were exposed to falls hazards from the supported scaffold.         GTO Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(c)(1) which was contained in OSHA inspection Number 992165 Citation Number 1, Item Number 2, and was affirmed as a final order on November 19, 2014 with respect to a workplace located at 2337 N. On or about April 18, 2017, at the front side of the building, an employee in an extensible boom lift basket did not have his body harness connected to a lanyard that was connected to the boom or basket and was exposed to falls greater than 6 feet.     GTO Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.453(b)(2)(v) which was contained in OSHA inspection Number 1185082 Citation Number 1, Item Number 1, and was affirmed as a final order on 12/23/2016 with respect to a workplace located at 806 S. On or about April 24, 2017, at the east stairs, stair landing between the 1st and 2nd floors, a toeboard was not present at the interior wall opening.   Employees at the first floor were exposed to struck-by hazards from objects falling through the unprotected edge.
