29 CFR 1926.501(c)(1): When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels: a. On or about April 24, 2017, at the south stair landing between the 2nd and 3rd floors, a toeboard was not present at a wall opening that was above an exit door which exposes employees to struck-by hazards from objects falling through the unprotected floor edge. b. On or about April 24, 2017, at the south stair landing between the 3rd and 4th floors, a toeboard was not present at a wall opening that was above an exit door which exposes employees to struck-by hazards from objects falling through the unprotected floor edge. c. On or about April 24, 2017, at the 4th floor, a toeboard was not present at a wall that was open to the stairs and which exposes employees to struck-by hazards from objects falling through the unprotected floor edge.

On or about April 19, 2017, at the front side of the building, a barricade was not erected at ground level where overhead metal framing parts exposed employees to struck-by hazards. Metal framing parts were thrown from the 4th floor of the building to the ground.
