29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder:    a. On or about August 28, 2014, On the front of the house, the 24-foot extension ladder being used to access the roof does  not have the ladder side rails extended to at least 3 feet (.9m) above the upper landing surface nor is the ladder secured at it's top to a rigid support and a grasping device, such as a grab rail, to provide assistance to employees in mounting and dismounting the ladder, exposing employees to a 9-foot fall hazard.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:    a. On or about August 28, 2014, On the front of the building, the 24-foot extension ladder being used to access the roof had a working length of 10.8 feet, and the horizontal distance from the top support to the foot of the ladder was 6 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are not protected by guardrail systems, safety net system, or personal fall arrest system, nor are employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a.  On a 6:12 pitch roof, employees conducting re-roofing operations are exposed to a 13 foot fall hazard in that they are not protected by a fall protection system.     To abate this violation, the employer must ensure that all employees wear appropriate fall protection or install a guardrail or safety net system when an employee works 6 feet or more above lower levels.      Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees form fall hazards.
