29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end: (a) Tom Clayton Erectors, LLC on or about August 11, 2014, at the site located on the 500 block of W. Market St. Tiffin, OH: The employer failed to ensure the employer fabricated scaffolding platform supported by a CAT TL642 extending boom forklift from which employees were working, extended no more than 18 inches beyond the supporting forks. Employees performing steel siding work from the elevated platform were exposed to falls of approximately 10 ft. as the 16 ft. long platform position on top of the supporting forks extended approximately 6 ft. on each side beyond the supporting forks.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) Tom Clayton Erectors, LLC on or about August 11, 2014, at the site located on the 500 block of W. Market St. Tiffin, OH: The employer failed to ensure the employer fabricated scaffolding platform supported by a CAT TL642 extending boom forklift from which employees were working was attached and secured to the supporting forks. Employees performing steel siding work from the elevated platform were exposed to falls of approximately 10 ft. as the 16 ft. long platform position on top of the supporting forks was not secured to the supporting forks.
