29 CFR  1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed, and the ladder was secured at its top to a rigid support that will not deflect, a grasping device was not provided to assist employees in mounting and dismounting the ladder.  Tom Clayton Erectors, LLC, at the worksite located at 235 N. Sandusky St. in Tiffin, Ohio:  On or about October 1, 2020, an extension ladder, used to access the roof of a commercial structure, was not utilized with a grasping device to assist employees in transition to/from the roof.  The ladder was not long enough to extend three feet above the landing service, exposing employees to fall hazards of approximately 21 feet.
