29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). a. On or about July 10, 2019, located at 30931 State Route 93 South in Mc Arthur, Ohio, an employee was in an aerial lift without a lanyard to be attached to the basket or boom, thereby exposing the employee to a maximum fall hazard of 30 feet.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about July 10, 2019, located at 30931 State Route 93 South in Mc Arthur, Ohio, employees were installing gutters from an aerial lift and from the roof of the building. An employee was not protected from falls while on the roof to collect tools, thereby exposing the employee to a fall hazard of approximately 30 feet.

The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head. a. On or about July 10, 2019, located at 30931 State Route 93 South in Mc Arthur, Ohio, an employee was installing gutters from the roof's edge while the chest strap was not fastened. This resulted in the attachment point on the employee's body harness to no longer be at the center of the back near shoulder level or above the head, thereby exposing the employee to a fall hazard of approximately 30 feet.
