29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   A)    Basic Grain Products, Inc., located at 300 East Vine Street, Coldwater, OH 45828: On or about August 11, 2014, the employer did not ensure that points of operation of the Sector A Puffer Machine (Rice Cakes Machine  18 banks, 5 stations in each bank, 90 stations total) were adequately guarded. Employees were exposed to pinch point hazards while operating the machine.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:   A)    Basic Grain Products, Inc., located at 300 East Vine Street, Coldwater, OH 45828: On or about September 3, 2014, the employer failed to ensure a clearance of at least three feet in front of the red electrical panel located in Sector F Puffing Room (240 Volts, 3 Phase). The electrical panel was blocked by a conveyor in front of it.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  A)    Basic Grain Products, Inc., located at 300 East Vine Street, Coldwater, OH 45828: On or about August 18, 2014, the employer failed to ensure that maintenance employees were provided with training on electrical safety-related work practices such as, but not limited to, arc flash/blast hazards and protective equipment for the hands, head, and body to protect against electrical hazards while working on or near energized or potentially energized electrical parts.  Employee was exposed to 240 volts electrical hazard while wiring the new auger system in the energized electrical panel (red panel that controls power to conveyors, augers, and exhaust fan), located in the Sector F Puffing Room, without locking the panel out and without proper electrical personal protective equipment.    B)     Basic Grain Products, Inc., located at 300 East Vine Street, Coldwater, OH 45828: On or about September 3, 2014, the employer failed to ensure that maintenance employees were provided with training on electrical safety-related work practices such as, but not limited to, arc flash/blast hazards and protective equipment for the hands, head, and body to protect against electrical hazards while working on or near energized or potentially energized electrical parts. Employees did not isolate all energy sources in order to control hazards associated with moving parts of the machines while performing activities such as, but not limited to, adjusting the puffer's back window in section 13-1. Employees were exposed to amputation hazards.    B)     Basic Grain Products, Inc., located at 300 East Vine Street, Coldwater, OH 45828: On or about September 3, 2014, the employer failed to utilize machine-specific energy control procedures to control hazardous energy of the Sector F Puffer Machine (Rice Cakes Machine).
