29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a. On or about April 24, 2015, the employer did not ensure that employees who thread the Triangle A machine (s/n 118063) were adequately trained when to use the safe application, usage, and removal of energy controls. Employees that threaded packaging into the machine were not trained to isolate energy sources to protect against the unexpected energization and/or release of stored energy during the process.

29 CFR 1910.147(d)(2): An orderly shutdown of machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage: a.) On or about April 24, 2015 The employer failed to ensure that energy isolating procedures for shutdown were performed prior to threading the packaging material in Triangle A machine (serial #118063) during the weekly setup. Employees performing this task in energized condition were exposed to caught in hazards from the rollers that feed the packaging material through the machine. Basic Grain Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(d)(2)), which was contained in OSHA inspection # 993601, Citation number 2, item number 1-b and was affirmed as a final order on 1/15/2015, with respect to a workplace located at 300 East Vine St., Coldwater, OH 45828. Basic Grain Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(d)(4)(i)), which was contained in OSHA Inspection # 98342, Citation number 2, item number 1-a, and was affirmed as a final order on 12/12/2013, with respect to a workplace located at 300 East Vine St., Coldwater, OH 45828.
