29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service: Inside lane of an overpass bridge: On or about September 4, 2014, employees renovating a concrete bridge used two chain slings to lift sections of the bridge concrete barricade and temporary barricade. The slings had damaged links and were available for use. This exposed the employees to the hazard of being struck by a falling piece of barricade.

29 CFR 1926.251(b)(1): Welded alloy steel chain sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and sling manufacturer: Inside lane of an overpass bridge: On or about September 4, 2014, employees renovating a concrete bridge used two chain slings to lift sections of the bridge concrete barricade and temporary barricade. The slings did not have identification tags attached and were available for use. This exposed the employees to the hazard of being struck by a falling piece of barricade.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Inside lane of the overpass bridge: On or about September 4, 2014, employees renovating an interstate overpass bridge had removed the concrete barricade and were preparing the bridge for repair and to construct a new barricade. No fall protections systems were in use exposing the employees to the hazard of falling over 6 feet. The PBX Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1) which was contained in OSHA inspection number 512919, citation number 1, item number 2b and was affirmed as a final order on November 6, 2012 with respect to a workplace located at Oklahoma City, OK.
