29 CFR 1926.20(b)(2):   Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On 9/11/2014, the sub-contractor/owner and four of his employees were repairing a roof of a residential building located at 22, 24 and 26 Tournament Tee Drive, OFallon MO.  The owner of Twins Construction Inc., controlling contractor for the sub-contractor, was at the site and did not conduct an adequate inspection of the job site, materials, and equipment.  The following violative conditions existed at the time of the inspection:  a)      Two employees were not wearing eye protection while using pneumatic nailers and hammers.  b)      Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  c)       Employees were using a ladder improperly to access the roof.  The ladder was                                                          not extended three feet beyond the landing or edge of the roof.  d)      Employees were carrying bundles of shingles and supplies up a ladder, which could have caused them to loose balance and fall.
