29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) Norfolk Southern Rutherford Yard, Harrisburg Pennsylvania - An employee did not have the lanyard of his personal fall arrest system connected the basket of the JLG 600S aerial work platform. The employee operated the aerial work platform 16 feet above the ground while he installed metal siding panels onto the building on or about September 8, 2014. Keystone Steel Structures LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 973170, citation number 1, item number 1, and was affirmed as a final order on August 25, 2014, with respect to a workplace located at 4 Lerchenmiller Drive, Northampton, PA 18067. Keystone Steel Structures LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 586282, citation number 1, item number 2 and was affirmed as a final order on September 27, 2012, with respect to a workplace located at 5100 Main Street, east Petersburg PA 17520.
