29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a) Aerial Lift - An employee screwing down the edge of the roof from an aerial lift was exposed to a fall hazard of approximately seven (7) foot six (6) inches while the lanyard was not attached to the basket, on or about May 7th, 2015.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.    Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.    Keystone Steel Structures LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 995104, citation number 2, item number 1 and was affirmed as a final order on October 20, 2014, with respect to a workplace located at 7150 Grayson Road, Harrisburg, PA 17111.
