29 CFR 1926.102(a)(1): Eye and face protective equipment was not provided when machines or operations presented potential eye or face injury:  a) LLG Construction, LLC, Grabill, Indiana:  On or about September 9, 2014, at the location of Lot #57, Goldenrod Lane, Perrysburg, Ohio, the employer failed to ensure that each employee was provided appropriate eye protection while engaged in nailing activities, such as securing roof sheathing to a residential structure with a pneumatic nailer, exposing employees to eye injuries from flying particles/objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) LLG Construction, LLC, Grabill, Indiana:  On or about September 9, 2014, at the location of Lot #57, Goldenrod Lane, Perrysburg, Ohio, the employer did not ensure that employees were protected from fall hazards while engaged in roofing activities. Employees were exposed to potential falls of approximately 20.2 feet to the ground below while applying roof sheathing.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) LLG Construction, LLC, Grabill, Indiana:  On or about September 9, 2014, at the location of Lot #57, Goldenrod Lane, Perrysburg, Ohio, the employer did not ensure that each employee had been trained in the recognition and control of fall hazards.  Employees exposed to falls of approximately 20.2 feet were not trained to use fall protection systems, which could result in permanent disability or death.
