29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  LLG Construction, LLC:  On or about May 9, 2016 the employer did not submit documentation certifying that abatement has been completed, for citations that became a final order on March 31, 2016.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.1053 (b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.      A. LLG Construction LLC worksite located at 14837 Five Point Rd., Perrysburg, Ohio:  On or about December 10, 2015,  the employer did not ensure the side rails of the extension ladder was extended at least three feet above the landing surface. Employees were exposed to falls while using the ladder to access the top of the exterior wall.

LLG Construction LLC worksite located at 14837 Five Point Rd., Perrysburg, Ohio: On or about December 10, 2015, the employer failed to ensure the top of an 6 ft A-framed stepladder was not use as a step by an employee. The employee was using the top of the ladder to access the ridge while installing rafters. The employee was exposed to fall hazards due to improper ladder use.

A. LLG Construction LLC worksite located at 14837 Five Point Rd., Perrysburg, Ohio: On or about December 10, 2015, the employer did not ensure employees were provided with proper eye protection while operating pneumatic nail guns when performing framing activities.  Two employees were exposed to potential flying object hazards while operating the pneumatic fasteners.    LLG Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102 which was contained in OSHA inspection number 995373, citation number 1, item number 1 and was affirmed as a final order on December 2, 2014 with respect to a workplace located at Perrysburg, Ohio.

Employees were exposed to a fall distance of approximately 10.00 feet.    LLG Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 which was contained in OSHA inspection number 995373, citation number 1, item number 2a and was affirmed as a final order on December 2, 2014 with respect to a workplace located at Perrysburg, Ohio.
