29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  On or about April 10, 2024 the employer LLG Construction, LLC at jobsite located at 1112 N. Cannon Creek Dr. Perrysburg OH 43551, did not have a plan or a program to identify job site hazards which assists employees in accident prevention and maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls and struck by hazards during job site operations, decreasing the probability of employee injury or illness.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about April 10, 2024 the employer LLG Construction, LLC at jobsite located at 1112 N. Cannon Creek Dr. Perrysburg OH 43551, did not have a fall protection training program in place to enable each employee to recognize the hazards of falling, and procedures to minimize these hazards. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about April 10, 2024 the employer LLG Construction, LLC at jobsite located at 1112 N. Cannon Creek Dr. Perrysburg OH 43551, did not ensure that each affected employee used appropriate eye or face protection when performing residential construction framing activities on a new build residential structure while exposed to potential struck by hazards (flying particles/debris) while using pneumatic nail guns and circular saws. One employee was exposed to struck-by/foreign body in the eye hazards while installing shingles while using a DeWalt brand circular saw (Model DCS575. S/N 068322). One employee was exposed to struck-by/foreign body in the eye hazards while installing soffit framing material while using a Paslode brand pneumatic framing nail gun.

Cannon Creek Dr. Perrysburg OH 43551, did not ensure employees were protected from falls while conducting residential framing construction activities on the second (roof) level of a residential new build multi family home. One employee was exposed to falls of 10 feet (from the eaves of the second level roof level) or greater to the ground below.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 10, 2024 the employer LLG Construction, LLC at jobsite located at 1112 N. Cannon Creek Dr. Perrysburg OH 43551, did not ensure the side rails of one aluminum extension ladder was extended at least three feet above the landing surface. One employee was exposed to falls from approximately 10 feet or greater from the top of the second (roof) level to the ground below while using a aluminum extension ladder to access the second (roof) level to perform residential framing activities on a multi-family home.
