OSH ACT of 1970 Section (5)(a)(1): : The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fall hazards when being lifted in personnel platforms with unprotected sides.  a) Style Crest Enterprises, Inc., Fremont, Ohio:  On or about September 16, 2014, an employee was exposed fall hazards while picking stock from racking systems while working from a personnel platform that was supported by a forklift.  The employee was elevated in a personnel platform (light yellow platform marked with 605) that was not equipped with standard guardrail and did not provide an alternate form of fall protection.   b) Style Crest Enterprises, Inc., Fremont, Ohio:  On or about September 4, 2014, an employee was exposed fall hazards while picking stock from racking systems while working from a personnel platform that was supported by a forklift.  The employee was elevated in a personnel platform (dark yellow platform) that was not equipped with standard guardrail and did not provide an alternate form of fall protection.   Among other methods, one feasible and acceptable method to correct this hazard is to follow the American National Standards Institute/Industrial Truck Standards Development Foundation  ANSI/ITSDF B56.1-2012 Safety Standard Low Lift and High Lift Trucks, Chapter 7.31.1 by providing a personnel platform that complies with the design requirement it sets forth, such as having standard guard railing system equipped on the personnel platform.
