29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) Style Crest Enterprises, Inc., located at 600 Hagerty Drive, Fremont, OH 43420: On or about February 10, 2015, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chucks of equipment such as, but not limited to, the Mighty Turn horizontal lathe machine (Model # ML-1880GL) located in the tool room. Employees were exposed to struck-by and amputation hazards. B) Style Crest Enterprises, Inc., located at 600 Hagerty Drive, Fremont, OH 43420: On or about February 24, 2015, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chucks of equipment such as, but not limited to, the Bridgeport vertical mill machine (Drive Serial # 2J-57424) located in the tool room. Employees were exposed to struck-by and amputation hazards. C) Style Crest Enterprises, Inc., located at 600 Hagerty Drive, Fremont, OH 43420: On or about February 24, 2015, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chucks of equipment such as, but not limited to, the Millport vertical mill machine (Model # 3VH, Serial # 7036) located in the tool room. Employees were exposed to struck-by and amputation hazards.
