29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):                                                                        The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site to identify existing and predictable hazards such as, but not limited to: employees exposed to falls from heights over 25 feet while installing roofing material, eye injury while using pneumatic nail guns, and foot injuries while cleaning roofing materials from the surrounding structure.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    On July 10, 2014, protective footwear was not used by employees walking on, and involved with cleaning up, roofing materials to prevent nails and other roofing materials from causing foot injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      On and before July 11, 2014, employees installing roofing materials on a residential home were exposed to falls ranging from 9 foot to greater than 25 feet high while working on roofs with pitches ranging from 2 in 12 (10 degrees) to 12 in 12 (45 degrees) without any fall protection.
