On or about September 24, 2014, the employee climbed the guardrail system of the man basket and did not have on a harness, exposing himself up to a 60 feet fall hazard.

On or about September 24, 2014, the employee painting the sign post from the truck mounted aerial lift man basket was not wearing a body belt and lanyard attached to the basket. He exposed himself up to a 60 feet fall hazard.
