29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Instance 1) At the jobsite, an employee standing at the edge of the four story building, was exposed to a 42 foot fall hazard, in that, fall protection equipment was not installed or utilized. Observed on or about 09/25/2014.    Instance 2) At the jobsite, an employee walking on the top plate of an interior wall was exposed to a 10'4" fall hazard, in that, fall protection equipment was not installed or utilized. Observed on or about 09/25/2014.      Rafael Total Services, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 991124, citation number 1, item number 1 and was affirmed as a final order on September 11, 2014, with respect to a workplace located at 1945 Noor St., Wesley Chapel, FL.
