Instance 2) At the jobsite, an employee walking on the top plate of an interior wall was exposed to a 10'4" fall hazard, in that, fall protection equipment was not installed or utilized. Observed on or about 09/25/2014.      Rafael Total Services, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 991124, citation number 1, item number 1 and was affirmed as a final order on September 11, 2014, with respect to a workplace located at 1945 Noor St., Wesley Chapel, FL.
