29 CFR 1926.502(d)(6)(i): Snap hooks were connected  directly to webbing, rope or wire rope:    a. On or about December 04, 2014, employees were exposed to the hazard of falling, approximately 40 feet, by having the snap hooks on their lifelines attached back to the rope portion their lifelines. The lifelines were wrapped around the wooden trusses and the snap hooks were attached back to the lifelines themselves.

On or about December 04, 2014, an employee was exposed to the hazard of falling approximately 40 feet, while doing framing work at a residential apartment complex. One of the lifelines that was being used was braced against the sharp edge of a metal framing bracket, while the employee was directly at the edge of the roof.

The employees were wearing safety harnesses and were attached to lifelines that had approximately 41 feet of slack in the lifelines.

Rafael Total Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 991124, citation number 1, item number 1 and was affirmed as a final order on Sept 11, 2014, with respect to a workplace located at 1945 Noor St. Wesley Chapel, FL, 33544.
