29 CFR 1926.1417(a): The employer did not comply with all manufacturer procedures applicable to the operational functions of equipment, including its use with attachments. On or about September 24, 2014, employees were exposed to caught between and struck-by hazards when Lee Mechanical Contractors, Inc. did not comply with the manufacturer operating instructions for job site travel of a crane. The employer did not ensure that a Link Belt RTC 8090 was properly prepared for travel as the boom was raised and operated over the right side while the outriggers were not extended. This condition caused the crane to lose backward stability and overturn. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
