29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected;     On or about October 1, 2014, the employer did not ensure periodic inspections of lockout procedures included a review of each authorized employee's responsibilities under the energy control procedures.  Employees perform lockout on mechanical power presses, grinders, welders, and auxiliary machines.  Energy sources include, but are not limited to; electrical, pneumatic, hydraulic, and gravity.

On or about October 1, 2014, the employer did not ensure press operators and press helpers were protected from the ingoing nip points of the Press #4 push feed machine tensioner rollers.         b. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1910.212(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315250266 , CITATION NUMBER 01, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 19, 2011 WITH RESPECT TO A WORK PLACE LOCATED AT 881 WAYSIDE ROAD, CLEVELAND, OHIO.
