29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about January 15, 2021, the employer did not ensure the maintenance employee, prior to repair and servicing the Model 4KG automatic grommet machine, implemented energy control procedures by; preparing the machine for shutdown [per 1910.147(d)(1) requirements], turning off the machine [(d)(2)], isolating the machine from the energy source [(d)(3)], affixing a lockout device [(d)(4)(i)], and verifying de-energization and isolation had been accomplished prior to repairing and servicing the machine [(d)(6)].

29 CFR  1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  On or about January 15, 2021, the employer failed to provide training to ensure affected and authorized employees understood the purpose and function of the energy control program and employees had acquired the knowledge and skills required for the safe application, usage, and removal of energy controls.  The employer engages in the service and maintenance activities on machinery and equipment including, but not limited to; power presses, grommet machines, and other machinery.

29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about January 15, 2021, the employer did not ensure periodic inspections of energy control procedures included a review of each authorized employee's responsibilities under the energy control procedures.  Authorized employees are required to conduct service and maintenance activities on the Model 4KG Automatic Grommet Machine requiring them to perform lockout/tagout.  Soundwich Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(6)(i) which was contained in OSHA inspection number 1251262, citation number 02, item number 001 and was affirmed as a final order on 9/28/2017, with respect to a workplace located at 881 Wayside Road, Cleveland , OH 44110.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about January 15, 2021, the employer had an employee receive an injury involving an amputation of the right thumb. The employer failed to report this injury timely in that the amputation was not reported until May 14, 2021.
