Instance 1) A man-made scaffold was built by the Foreman, on the roof and in front of a dormer, where no training, or experience in scaffold erection had been obtained. A qualified person did not inspect this scaffold.    Instance 2) A second man-made scaffold was built by an Employee, on the roof and in front of a dormer. A qualified person did not inspect this scaffold prior to use..

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    Location: Jobsite    Employees were exposed to fall hazards on an unguarded scaffold, above 10 feet, where Employees accessed the roof, and were working from unguarded scaffolds with no fall protection.    Instance 1) The Foreman was working from, and accessed the roof from the unguarded scaffold to install trim alongside a dormer roof edge.    Instance 2) Another Employee was working from another unguarded scaffold to install trim alongside another dormer roof edge, on the opposite side of the building.

Location: Jobsite    Employees were exposed to fall hazards greater than 6 feet, while framing the second floor, where a balcony edge and three wall openings were not properly guarded with a midrail.    Instance 1) The second floor balcony edge was not properly guarded with a midrail.    Instance 2) Three wall openings, two on the first floor and one on the second floor, were not properly guarded with a midrail.

Instance 1) The Foreman was working from the roof, attaching trim to the dormer, and was not using any form of fall protection when he fell to the ground.    Instance 2) Employees were exposed to fall hazards at unguarded wall openings on the second floor, where no guardrail system was used to prevent accidental falls.
