29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe: At the jobsite located at 41 N James St. Kansas City, Kansas, employees were exposed to struck-by hazards. Employees engaged in disassembling a P-14 conveyor were exposed to crushing and/or struck-by hazards while moving material with defective nylon slings and metal hook latches. Employees were exposed in that the employer did not remove the defective equipment prior to being placed in-use.
