On or about October 2, 2014, an employee operated a Hitachi nail gun to perform decking activities without eye and face protection exposing him to the hazard of being struck-by flying objects.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system. On or about October 2, 2014, employees performing decking activities on the roof of a 3 story apartment building were not provided fall protection and exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.
