OSH ACT of 1970 Section (5)(a)(1):     Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to the hazard of being struck-by:    a/  Inside of the Painting Department, employee(s) were exposed to a struck-by hazard due to a possible unplanned collapse of two metal, shelving units, each three tiers high, that were not anchored.     One method of abatement to correct this apparent hazard is to bolt or otherwise secure both shelving units to the concrete floor as required by ANSI MH16.1-2012, paragraph 1.4.7.     DISCLAIMER:    a/  The employer is not limited to the abatement method(s) suggested by OSHA.  b/  The employer is responsible for selecting and implementing an effective abatement method.

29 CFR 1910.23(a)(5): Infrequently used pit or trap door floor opening(s) were not guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening was not constantly attended by someone or was protected on all exposed sides by removable standard railings:    a/  Inside of the Fabrication Department, a floor opening measuring approximately 113"L X 23"W X 9.5"D was unguarded.  This unguarded floor opening was located adjacent to the east end of the Pacific Press Brake, m/n K200-1845, s/n 9007.    IN THE ALTERNATIVE:    29 CFR 1910.23(a)(7):  Temporary floor opening(s) were not guarded by standard railings or constantly attended by someone:    a/  Inside of the Fabrication Department, a floor opening measuring approximately 113"L X 23"W X 9.5"D was unguarded.  This unguarded floor opening was located adjacent to the east end of the Pacific Press Brake, m/n K200-1845, s/n 9007.

29 CFR 1910.23(e)(1): A standard railing did not consist of top rail, intermediate rail, and posts, and did not have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level:      a/  Inside of the Production Area, the railing along the open edge of a platform over the Age Oven consisted of a top rail only.  The vertical height of this top rail was approximately 35 inches above the platform.  The height of this platform was approximately 11 feet above the concrete floor.

Park Road, Union, MO 63084.
