29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    Jobsite, on or about October 07, 2014, employees were using extension cords without ground prong attached throughout the jobsite, exposing employees to electric and shock hazards.      a) Jobsite, on or about October 07, 2014, employees were using a red three-outlet extension cord with a missing ground pin that was attached to a yellow/black extension cord with a missing ground pin to power a Makita circular saw.      b) Jobsite, on or about October 07, 2014, employees were using a red three-outlet extension cord with a missing ground pin attached to a yellow extension cord to power a Makita circular saw.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Jobsite  417 Peachtree Drive, Rincon GA - On or about October 07, 2014, employees used an aluminum extension ladder without the side rails of the ladder being extended at least 3 feet above the upper landing surface nor was the ladder secured at its top to a rigid support to gain access to a second working level while installing ply-wood panels, exposing employees to fall hazards of about 11.5 feet onto the ground.     ABATEMENT CERTIFICATION IS NOT REQUIRED.
