29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Roof - On or about August 26, 2015, an employee installing floor joists was exposed to fall hazards up to approximately 12 feet onto concrete and earthen ground because the workers were not fall protected. Jose Alberto Callejas-Cruz was previously cited for a violation of this occupational safety and health standard or its equivalent standard [(1926.501(b)(13)], which was contained in OSHA inspection number 999051, citation number 1, item number 4 and was affirmed as a final order on November 19, 2014, with respect to a workplace located at 417 Peachtree Drive, Rincon, GA 31326.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Front Elevation, Left of Front Door - On or about August 26, 2015 employees used an aluminum extension ladder to gain access to the roof of the building without the side rails of the ladder being extended at least 3 feet above the upper landing surface nor was the ladder secured at its top to a rigid support, exposing employees to fall hazards of up to approximately 12 feet onto concrete and earthen ground. Jose Alberto Callejas-Cruz was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1926.1053(b)(1)], which was contained in OSHA inspection number 999051, citation number 1, item number 5 and was affirmed as a final order on November 19, 2014, with respect to a workplace located at 417 Peachtree Drive, Rincon, GA 31326.
