29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the job site: On October 8, 2014 employees using five gallon buckets of roofing adhesive did not have a fire extinguisher available, exposing employees to burn hazards while using flammable liquids. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On October 8, 2014 employees were not protected from a fall while roofing a commercial building with a flat roof, exposing employees to a fall of over 25 feet above grade. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
