29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On August 6, 2015, employees were exposed to a fall hazard over six feet due to unguarded skylights while working on the roof of a commercial building and the employer did not ensure skylights were properly guarded. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On August 6, 2015, employees were exposed to a 20.9 foot fall while roofing a flat, commercial building and the employer did not ensure employees were utilizing fall protection. MAC CONSTRUCTION & ROOFING, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926. 501(b)(10), which was contained in OSHA inspection number 1000059, citation number 1, item number 2 and was affirmed as a final order on November 19, 2014, with respect to a workplace located at 301-305 W Main St. Saint Charles, IL 60174. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
