29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about June 11, 2019, employees were exposed to a fall hazard over 6-feet due to unguarded skylights while working on the roof and the employer did not ensure skylights were properly guarded. MAC CONSTRUCTION AND ROOFING, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(4)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1083105, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 22, 2015, WITH RESPECT TO A WORKPLACE LOCATED AT 303 GRAND AVE, BENSENVILLE, IL 60105.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone: On or about June 11, 2019, employees were exposed to fall hazards of approximately 16-feet above the flat roof surface below while performing safety monitoring of the employees tearing off a membrane roof because of the lack of fall protection. MAC CONSTRUCTION AND ROOFING, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD NUMBER 29 CFR 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1000059, CITATION NUMBER 1, ITEM NUMBER 2, AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 19, 2014, WITH RESPECT TO A WORKPLACE LOCATED AT 301-305 W MAIN ST, SAINT CHARLES, IL 60174.
