29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about October 16, 2014, and at times prior thereto, at the corner of 2000 Grant Ave., two employees were observed working off a JLG T350 Tow-Pro Boom Lift without any form of fall protection and were exposed to a fall hazard of greater than ten-feet.
