29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about March 03, 2016 and at times prior thereto, an employee was working on an aerial lift without any form of fall protection. The employee was exposed to fall hazards of greater than ten feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about March 03, 2016 and at times prior thereto, employees were replacing a sign on the side of a commercial building. They were using an extension ladder to access the roof and it did not extend 3 feet above the landing. The employees were exposed to fall hazards of greater than six feet to the ground below.
