29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.) Construction Site/6310 Old Station Road North, East, PA, 16428  Excavation Area: On or about October 16, 2014 and at times prior thereto, employees were not wearing hard hats while working in an excavation. Employees performed work activities in the excavation that included but were not limited to dismantling form work, form shoring, and form work scaffolds.  Employees were exposed to falling objects from personnel who were working aloft on ladders and from the form work scaffold.  In addition, employees were exposed to falling objects from lose rock/debris that were present on the sides of the excavation.

Construction Site/6310 Old Station Road North, East, PA, 16428  Excavation Area:  On or about October 16, 2014 and at times prior thereto, employees were potentially exposed to cave-in hazards while working in an excavation performing activities that included but were not limited to the erection/dismantling form work/form work scaffolds and concrete construction for a retaining wall.  The excavation consisted of near vertical walls and ranged in height from at least 5-feet to 20-feet. Employees were not protected against cave-in hazards by any form of protective system.

The water had accumulated in the bottom of the excavation.  Personnel were potentially exposed to cave-in hazards while working in the excavation performing activities that included but were not limited to the erection/dismantling form work/form work scaffolds and concrete construction for a retaining wall.  The excavation consisted of near vertical walls and ranged in height from at least 5-feet to 20-feet.

Construction Site/6310 Old Station Road North, East, PA, 16428  Excavation Area: On or about October 16, 2014 and at times prior thereto, employees were potentially exposed to a fall hazard of at least 5-feet from a step-ladder which was utilized for purposes other than which it was designed by the manufacturer.  The step-ladder was utilized improperly in that it was folded and utilized as a straight ladder, its top-step and top-cap were climbed upon, and its side rails did not extend 3-feet above the side of the excavation.  Employees utilized the step-ladder for activities that included but were not limited to accessing and exiting the excavation.

The step-ladder was folded, utilized as a straight ladder, and its top-step/top-cap were positioned just below the edge of the excavation.  The step-ladders side rails extended 0-feet above the top of the excavation. The condition exposed employees to a fall hazard of at least 5-feet while they were utilizing the step-ladder.

Construction Site/6310 Old Station Road North, East, PA, 16428  Excavation Area: On or about October 16, 2014 and at times prior thereto, employees utilized the step-ladder for activities that included but were not limited to accessing and exiting an excavation.  The step-ladder was folded, utilized as a straight ladder, and its top-step/top-cap were positioned just below the edge of the excavation.  Personnel were stepping on the ladders top-cap and top-step when accessing and exiting the excavation. The condition exposed employees to a fall hazard of at least 5-feet while they were utilizing the step-ladder.
