Suram Construction Company, Inc., 6310 Old Station Road, North East, Pennsylvania 16428, third floor, on or about March 3, 2016 and times prior thereto: A flexible electrical extension cord was spliced into a non-metallic sheath cable. The flexible cord was equipped with a cord cap that was plugged into an electrical receptacle. The electrical circuit was powering equipment including, but not limited to, temporary lighting on the fourth floor of the structure.

29 CFR 1926.405(a)(2)(ii)(B): Runs of open conductors used as temporary wiring branch circuits were located where the conductors would be subject to physical damage, or the conductors were fastened at intervals exceeding 10 feet: A.) Suram Construction Company, Inc., 6310 Old Station Road, North East, Pennsylvania 16428, third floor, on or about March 3, 2016 and times prior thereto: A flexible electrical extension cord was spliced into a non-metallic sheath cable. The flexible cord was equipped with a cord cap that was plugged into an electrical receptacle. The electrical wiring was powering equipment including, but not limited to, temporary lighting on the fourth floor of the structure. The wire splice was 64-inches from the floor. Employees utilize equipment including, but not limited to, fiberglass ladder for work operations, which could potential contact and cause physical damage to the splice.
